Missed Appointment Policy
As a courtesy to all our patients, we ask that cancellations or rescheduling requests be called into our office at least 24 hours prior to scheduled visits.
There will be a missed appointment fee charged for appointments cancelled without the proper notice. The fees are as follows:
Physician Appointment $25
Audiology Appointment $30
ENG/VNG Appointment $100
Three missed appointments in any twelve-month period will result in being dismissed from the practice. Patients will not be seen until all of the missed appointment fees are paid in full.
Please note that cosmetic surgery consults require 72 hours notice of cancellation. A fee of $75 will be charged for cosmetic consultations that do not give proper notice of cancellation.

Tuesday-Friday: 9:00AM - 5:00PM
